The big spoiler is that Venom Snake is NOT Big Boss but actually the medic who removed the bomb from Paz's stomach. Through plastic surgery and hypnotherapy and suggestion. he thinks is he is Big Boss, while the real Big Boss was Ishmael, the guy in bandages in the hospital. He goes off to make Zanzibarland while Venom aka Big medic makes Outer Heaven. It is Big Medic that was killed by Solid Snake in the Outer Heaven uprising, not Big Boss. The other ending is a cut mission that is on the collectors edition as a video. It is the culmination of the Eli and Venom Snake story. It was unfinished and can be found on youtube.
