Kieslowski's Red with Irene Jacob and Jean Louis Trintignant. One of my fav films, and there are a number of parallels with Miele. But also keep in mind that Red is far more complex, whether on its own, or as part of an even more complex film trio- which all come together in the end of Red.
A much simpler (but progressive for its time)Hollywood film , with many humorous moments, is Harold and Maude, where the main characters are a reversal of the old/young in Miele.
Goodbye Solo also resonates. A small independent U.S. film with tremendous performances.
Kiarostami, the great Irani director, made A Taste of Cherry, where a stranger seeks a stranger to help him die.
Decade ago I saw The Ballad of Narayama , told as a Japanese folk tale about family duty and the old sacrificing themselves for the next generation.
