Did Milo end up dieing at the end? It wasn't very clear...

I don't think he did. Duncan just maimed him, symbolizing I suppose that he has overcome his problems. If he had died Duncan would be brain dead like his dad.

It's very clear that no, Milo did not die. If he had died, Duncan would have been "lobotomized" like his father is after Milo kills Ralph.
