last line of the movie

The last shot at the end of the film shows John driving away in his car and there is an offscreen voice of John's girlfriend saying "Are you okay John?"

It doesn't appear that sh is in the car, so does that line have any significant meaning that alludes to a twist ending or not?

When you hear her ask "John, are you OK?" you also hear a shovel scraping dirt and the scene has faded to black. This implies in the end John is now buried and the GF is either digging him out or piling on dirt. Makes no sense to me unless the director/writer is just effing with our heads.

He killed sixteen Czechoslovakians. Guy was an interior decorator.

Oh I didn't realise that, hmm, maybe it was like a metaphor for him being able to finally bury his past that had been troubling him for so long... With his dad killing himself in front of his eyes as a kid

Just saw this movie. I thought that the last time meant that he was thinking of when he buried the guy and she was asking if he was ok. That it bothered them more than it did the killer.
