Unless I missed it too, they never show how the adoptive mother dies. They do show her paying no attention to the road and crossing the center line when she's driving. I'm thinking car crash. From what I understand, the fifth nail is a nail to the heart to stop the ritual dad started.

I think the spirit of Faith took over when his aunt (aka adoptive mom) was in the hospital. That's what the dead crow bird was telling her to do, the feather she picks up in the scene is the one they put on Bryan in their ceremony for eternal life. With all of the drugs that his aunt was on it would make her an adequate person.

I think, given the wound to the back of her head, she was in an accident on the way to get Brian. That's when she died.
