Presumably went home, spent the night, came in the next morning, and nobody said a word to him about it?

Evidently it's not against the rules to "go out to get some air" (even for several hours) during a challenge. If he'd been verbally or physically abusive toward someone prior to leaving, I'm sure he would have been DQ'ed from the competition.

If he hadn't decided to come back, I wonder if they would still have eliminated a designer after the runway? That wouldn't seem fair but a lot of things on this show don't seem to make much sense.

I thought this might be an issue as well, especially since part of Keith being disqualified from season 3 (including having the pattern making materials) was disappearing from the production for hours at a time. Apparently Ken stayed in contact with the production team when he left the workroom and didn't return until the next morning.

Unless more was shown, contestants aren't just allowed to leave. This is due to spoiler alerts. A chaperone would have had to go with him. Or they were just desperate to have a contestant back, because they have an agenda.

Most likely a chaperone went with him. He may have went back to the hotel, where production is always on hand. If he walked away from production he would have had access to the internet and other advantages that the rest of the designers wouldn't have had access to (internet access was also a big part of Keith's disqualification when he pulled his disappearing act in Season 3 of PR).
