He's a rough bail-bondsman. They don't tend to be polite. The joke is on HIM, not the target of the insult. It's grounded in the fact that 'Sandy' is a unisex name, so it DOES have justification, whether or not you think it's acceptable (in real life, i don't, in movies, I do.)
I would never use that slur, but it is a fact that there are mean people out there who do… Movies have a right to depict this, unless we just want to go ahead and ban EVERY insult, which is absurd… (I'm disabled, you can call me a cripple… I hope you don't, but it's your right of free speech).
"Your mother puts license plates in your underwear? How do you sit?!"
