This is probably a stupid question but when you see her holding Zoey's baby I take it this is a dream

It's her vision or dream as she passes away I believe.

I think it's a dream because Tessa died before she got chance to meet Zoey's baby. Tessa wanted to meet Lauren more than anything. The nurse told Tessa that she would dream a lot over her last few days. Such a sad, yet inspiring, movie.
