When she was walking down the hall of the hospital to her sister's room, in the penultimate scene, I thought OMG, we are going to find out that she is the crazy one and her sister is all in her mind. Was relieved to see her twin sitting in the room with the blonde short haircut.

It would make great ending, but alas, they'd spoiled the cutest thing. Just look at the bunch those weird filmmakers (in the Extras on DVD for ex.). They had no idea what they must do.
Yes, part of me was relieved, but if it ended with her being psychotic after all, it would have elevated the movie. but then again it would have been a cliche as well. I'm of two minds.
shareI would have preferred it was in her mind, and there was no twin. This fairly dire movie needed a twist.

I think you captured it perfectly. Sadly, my viewing partner while watching this movie bailed on me because he could see the end coming and that there was no really point to the movie. I kept holding out, wishing for some kind of twist. This would have made the film much more interesting.
