I get most of this but why is....(spoiler)

Why would there be a chalk written note, "this is the first time"? How would he know he is in a time loop to do that unless this is all about mental illness? And if it is about the schizophrenia, how would we know/see the interactions he was not involved in?
I also don't get why Michael 1 would just go on a complete killing spree that included Sarah and her boyfriend in the RV. I get that he was angry at those who betrayed him, but they never got there. AND they were lost too so how did Michael find them?
I don't need a movie to tie everything together in a neat package; I just hate when I can't reason out the little details for myself. Thanks to anyone who can explain this to me!

remember the part near the end when he warns himself that they are going to try and harm him and lock him up. he tells himself that they will lock him up two times. he writes "this is the first time" so that his other self will read it the first time they lock him in the room in the mine.
