i also thought it was weird. but while the leadrolenurse clearly had no problem whith removing her undergarments, did you notice that she went towards the shower whith her stockings on ? must be a thing ;)
shareThe film was taken off the director and re-edited by the studio, you are seeing an R rated cut, the original scene probably had the actress naked or was shot differently to make it look like she was naked.
The original cut of the film would have been much sexier, much bloodier and unrated.

The version I saw online when it came out had her completely nude in the shower with her butt clearly visible up close, no panties (CGI or otherwise). There must have been something happening after that...

Dis you ever think that the actress had a no nudity clause in her contract? Maybe that is the reason. And actor has a CHOICE to do nudity or not, and they can put that clause in her/his contract.

I, too, thought it was pretty weird. People just don't shower in their underwear.
They don't have to show her naked at all but showing her showering in underwear was just odd.
