Why do they do it?

I don't want to sound blood-thirsty or anything, but it happens in so many movies where the hero (per se)has a gun aimed at the person who is terrifying him or her and there is a pregnant pause or conversation whereby the villian invariably escapes to exact more pain.
Supposedly, if the trigger is pulled, the movie is over, I guess.
