So we get the story at the beginning, about how he saw some smoke, or whatever, the night his sister died.
And that he thought the minister spoke nonsense to his mother about what happened.
Later on, he seemed to have some realisation about what had really happened that night, and what the minister had really been talking about.
So what really happened that night, and what did the minister say that explained it?
Nothing. That's why the script sucked so bad. Nothing was ever explained.

shareShe died of natural causes. He thought he should have been able to stop it somehow, and had felt guilty ever since. When he realized the minister had been saying (to his mother) that there was nothing that could have been done for his benefit, knowing he was listening, finally the unwarranted guilt lifted.
What he saw, whether that was his imagination or not, that part is left open.
