...staff locked in the cages? If he himself was an escaped patient, why would he be sympathetic to them? He was surprised and/or offended when he learned about Silas and Co. when he was no better.

Based on the scars he shows and the back story he shares, I presume he cared because he did not like to see suffering in others. Even if those others were utter bastards. He cared about being humane.
