Possible plot hole

One thing in this movie looks like a plot hole to me. When the young doctor is first given a tour in the asylum by Ben Kingsley, the Kate Beckinsale character is presented to him as one of the patients. Why was Kate Beckinsale not pretending to be a stuff member, given that she was one of the saner ones in the asylum? It would have made sense for Ben Kingsley to present Kate Beckinsale as the head nurse or something.

Of course, the movie could not go that way because then the young doctor would have realized immediately that something is amiss. But Kingsley did not know that the doctor had met Beckinsale before, even Beckinsale herself did not remember that. So there was no logical reason for them to have Beckinsale play a patient. It's not a big deal, of course. Just something I noticed.

Can't have a nurse with her mental disorder though.

She doesn't like to touch or be touched by Men she doesn't know very well. So how can she be a Nurse with a bunch of psychos, including rapists that would definitely try to feel her up?

We might make the same point with Millie--she seemed to have some sort of hypersexual disorder, and we do see her encouraging a man (Finn) who had attacked her. She runs the risk of outing herself as a fake nurse as well, but for the opposite reason as Eliza! I think the OP makes a good point that this decision is one only screenwriters would make, not the characters in their world.

Of course, we could imagine Eliza refusing to play a nurse because she was uncomfortable, yet she seemed happy to follow Lamb's orders to keep the new system running.
