The way I see it, it can be both ways. Either he really did get into a full-blown delusion or everything was real.

I just want to know what you guys think.

The first thing that happens for the shift back to the alien concept is that she sees the hologram, and the woman from joes past comes after her, so we are left to assume its real. If it's delusion, we would only see the alien realities from his perspective.
