Marco shoots the gas tanks, causes a massive explosion and gets blown back down the corridor, then a few minutes later, he is fine and talking to Peter in the office. Then when Peter 'confesses' to Kasumi, he doesn't actually say anything specific to make her question why he did what he did, so why did she get so upset? Was it just because he lied about being a patient? Was it the poor subtitles confusing me? Did I miss something here?

I think that is all just the bad writing and randomness that is a mark of many bad anime. The Japanese don't seem to think it is always necessary to make clear transitions and coherent story. I haven't finished the movie and stopped shortly after the scene you mentioned. It just became too random and stupid and there wasn't a single likeable or interesting character. In fact, I felt like punching them all to death myself.
