Does the portrayal of mental illness with in this film show through to anyone or is it pushed to the background or taken advantage of?

Sort of pushed to the background. Its hinted at but never outright stated, therefore being one of those things you have to read the book to comprehend completely what's going on.

To me it seemed obvious that he had mental health difficulties- brought on by being abused as a young child and not having anyone protect him, mainly.

It was. His aunt was abusing him and he wanted her dead. The car accident made him guilty.
