It's a psychological trauma buried as a Hollowface.. his ex-con father was released from jail, came looking for his son, and in that night there was an accident.The father fell to his death, into wet cement and basically died there. The boy came up with the Hollowface to cover it up. It's like when your parents are fighting and you're imagining that you are in the Disneyland lol. (from the simpsons episode: that's the best example i could think of right now)
So to sum it all up, The kid (now Mias father) buried his fathers death in story and a character named hollowface. Now how did that transferred to his daughter i can't put my finger on, maybe through genetics and DNA thing? She only found note, there is no way any kid would go crazy reading a boogeyman story on an old paper, start seeing weird stuff afterwards.
Overall fine movie, kept me interested throughout all of it.

Your comment sums up the movie well, imo. We can see the glimpse of psychological trauma that will be revealed by the end of the movie when Clive Owen's coworker in hanging from the building and Clive holds him by the hand. At that moment he sees the coworker's face as shapeless, just like Holloface's. The situation is reenactment of a father's hanging from the balcony before the fall that kills him. It probably triggers the long forgotten childhood memories and maybe this is why he starts seeing Holloface again. Only in the last 5 minutes of the movie coworker's shapeless face starts making sense.
Imo this is a movie that's horror until the very end when we start realising that what we've seen is not a horror at all but a psychological drama.
