I think my game had a glitch possible spoilers

So I have a ps4 I bought this in the store. Last night I beat the game. I kept dying as Norman in Mad Jack but I kept playing it until he survived so I continued the game. I also made it out alive for Fish Tank but the chapter Solving the Puzzle was skipped and Norman never came to the warehouse. I read that only if he did not die in Mad Jack which he didn't. Thoughts?

It sounds like a glitch or maybe you reloaded the wrong save file at some point? If Jayden is alive, you should always get the final puzzle.
