what does tommy lee mean at the end?

who is he talking about? god, devil?
i don't get this last part. if he's an atheist then who is he referring to?

He's talking about Death.

Death, metaphorically. He's not saying death has a personification, but that he yearns to embrace his demise. He earlier states that the only belief he has left to hold on to is the doom of man after a short period of suffering. The ultimate nullification death would be a release to him and so he is eager to find succor at its breast.

In some cultures the personification of death is shown as a skeleton wearing a black robe and carrying a scythe. The scythe severs the soul from the body so the soul can move on. Hence the line, 'his bony cheek' he quite literally means the skull of death, and the 'dry ancient ear', is just strengthening the allusion.

he is talking about his brother, proably. The brother is obviously sick, in some kind of hospital.
