If you watch the scene again, you'll notice it's Marcela who is talking with the old lady and not the boyfriend.
My guess is that after Marcela's suspicions that Nelson is cheating on her, she asks the old lady who confesses to her that in fact Nelson is cheating on her and the old lady seems to be crying out of guilt for not telling Marcela before.
I thought Nelson was cheating Marcela with one of the girls who sells the flowers and the old lady talking to Marcela in that scene is the girl's mother.
My guess is that the girl is pregnant that's why he had an argument in the phone with her when Marcela is leaving.
Good guess! It could be the girl's mother that Marcela is talking to. And I got the same impression that the reason the girl was so upset on the phone was because she was pregnant. At least now Marcela could leave without the guilty conscience.
