Probably Real: They have to sleep on location. They have to work. They get emotionally involved in events because thousands of years of evolution can't simply be turned off even when you know things are ultimately fake.
Honestly presented as fake (never was meant to be in question): They are surrounded by cameramen. They are instructed on various projects they have to accomplish. Marauders are actors.
Obviously fake (though not necessarily intended to be known): The marauders aren't just waiting around to attack if the team wanders out of their area. Every venture out of their local area is scheduled in advance, items are placed in advanced, actors are stationed in advanced, participants are probably not aware of what they are to find so you see them genuinely surprised even though most of it is staged. Items necessary to complete the projects are placed on the grounds before or at the time of project to be found.
Deceptively fake (the show tries to make this seem genuine): Everything they eat. I don't think they're slipping the participants steaks every night, because they obviously lose some weight, but they aren't eating turtle. That's why you never saw the turtle even dispatched, let alone eaten. The show seems to draw a line. They have/allow the participants eat some rough foods, but doubtful they actually managed to stretch 1 box of macaroni for 2 weeks. They are given enough food to work with. I believe the weight loss is probably more to do with them having to build crap and walk around all day. Everything to do with the end of each season is flagrantly fake, but they don't state it to be so.
Writers, producers, the director, makeup, wardrobe, are all on set all day making the models pretty with perfect hair and following people around telling them things to do. I believe this has the side of effect of making the participants "blind" to marauder attacks. They are so used to seeing various people slinking around, dodging the camera, running about, setting things up, etc. that when the marauders show up they don't even pay attention until the marauders make it obvious they aren't crew members.
Might be fake: Crew help build their projects. Nothing is a surprise, schedule known in advance, they don't actually have to live in their area 24/7. Maybe they can leave, make phone calls, hang out in a lounge, etc.
All the interviews I've seen taken into consideration, this is not an easy show. They aren't pretending to be hungry or tired. That is chief among the "reality" of the show. They are hungry, tired, and constantly tasked to do more without a day off. In that regard, this is a realistic survival show. There's things they could do to make it more real and less cheesy. But I enjoyed both seasons because they were ultimately fun and not entirely deceptive.
Of course, even if a show is 100% real. No assistance, no staging, completely hardcore, doesn't mean anyone will respect it. Out of the Wild is just about the most hardcore real survival show imaginable and it has basically no traction. Survivor on the other hand, 90% fake is revered for all time.
