By that point in the film Itekomo and Murase were out of the picture. The Chairman had promised Otomo that he could take over Itekomo and Murase's turfs. However, at this point he promises Ozawa (Itekomo's underboss) said turfs so long as he disposes of Otomo and his crew. At the same time it is Kato (the Chairman's right hand man) who orders the grenades thrown into Otomo's restaurant. Kato has also been promised the turfs of Itekomo and Murase when everything is resolved. But then there is another twist to the tale as one of Mizuno's executioners says something along the lines of "Ishihara says hello" before killing him. Then in the final scene we see Ishihara sat with Kato (who has now become the Chairman after killing both Ozawa and the Chairman). Based on this I think it's obvious that Kato and Ishihara were responsible for Mizuno's death.

As for Otomo, he's killed by Kimura in prison. Kimura had a grudge to settle with Otomo anyway after Otomo slashed his face. By this point I think it's safe to assume the Yakuza don't care if Otomo lives or dies and it is a personal thing between Kimura and Otomo. I don't think the corrupt police were directly involved.

I really enjoyed the film but by the end it was obvious the Chairman could have just sent in his men in the first instance to wipe out Otomo, Itekomo and Murase's crews. Rather than going to all the trouble of playing them off against each other.

Was there a reason they subjected him to such a violent death when they could have just shot him in the car park? Was there some personal grudge between Ishihara and Mizuno that I missed (it was hard to keep track of who was who, what with all the double and triple-crossings).

The sub's in the version I watched (I don't speak Japanese) came up as, "Ishihara says hello."
I think the point of the novel, violent death was more Ishihara's way of saying to Kato, "Don't cross me," than anything else.

By Ishihara's reaction to a certain question in Otomo's office, I was also unsure if he'd been skimming off the top of the drugs and casino the whole time anyway, hinting at him looking out for his own affairs, and his betrayal later on not being out of character.

As for the Chairman not sending his men to wipe out three gangs from the get-go: why would he?
The whole point of the film was the Chairman feeling threatened by the alliance of Otomo's sworn-father and Murase, so he wanted to destroy it. By pitting them against one another and making Otomo do the dirty work, he could simply take over EVERYTHING as a bonus, and without implicating his gang of any crimes or immoral behaviour (which is important if he, himself, has a Godfather with a code of honour to live by and whom to pay dues).

Still, the most troubling thing about the whole film was how under-par it was. I'd heard years back that it was a return to form for Kitano and I only recently got the DVD. I'll have to say that it's good I found it in a bargain basement sale and didn't pay full-whack from YesAsia when I actually meant to buy it, several years back.
