So Kay died from his cancer, but how did Cream die? I know it was the dentist that wrote the song, but there was nothing in the movie that indicated how she died....? Anyone have any thoughts or ideas?

This is such a ****HUGE SPOILER**** I hesitate to write it. She chose to follow him, willfully ... if you get my drift ... and tells him (on the tape playing) not to be mad at her, not to cry, when he sees her again.

Right. And the reference to Romeo and Juliet at the beginning of the film forshadows it.
