The way I see it, at least he's talking. Doc Drew mentioned emotions would start to run high as withdrawal set in. That's happening. Unlike, James who is stoic (to this point), and Kari Ann, who is avoiding her issues, Duncan is facing his demons and giving words to his feelings. It's ugly, but part of the process.
Another interesting thing about Duncan - the psychiatrist accused him of revisiting the abuse to his inner child every time he acted out homosexually. This came as an epiphany to Duncan. It seems like the psychiatrist is not just trying to cure Duncan of sex addiction, but homosexuality itself. I don't think that's wrong. In some cases - not all - I do believe homosexuality is a psychological disorder. Duncan may be an example.

well i think his homosexuality most likely came from the abuse he suffered, but i doubt that's something that can be 'cured.' he will undoubtedly always be attracted to men, but the goal of this therapy is to teach him how to have a healthy relationship. the girls in the house were also abused by men, and have been driven to unhealthy relationships because of this, that doesn't mean they can just become lesbians.

I thought the shrink was enlightening Duncan on why he goes for certain guys and sexual experiences. Not trying to change his homosexuality! "Acting out" meant recreating the trauma itself. He can still have healthy relationships with guys.

Acting out like this can be part of the rehab process. If he had not had this experience, he might not had the breakthrough he had afterwards. Anybody working with recovering addicts has to expect to deal with this sort of behavior.
