So in the end of the movie we see Tom has continued traveling instead of returning home. So are we to believe this doctor left his practice and life behind and is now going to spend the rest of his days hiking around the world?

Relax and see the world. He didn't seem unhappy with his life in general.
This is my thought too. He is now "living a life" as his son told him in the beginning of the film.
Maybe he did just travel, but that's not the way I interpreted it.
