The cut out faces of the artwork

On the print that I saw, there was a non-dialog subtitle pointing out that the cut-out face was that of a little boy (helpful, given the slightly androgynous nature of the drawing). Later, when the shop owner had his wife tied up and her face stuck through the cut-out, he berated her for not giving him a son, and said something about her having to pretend to be his little boy.

I'm not sure just how kinky we were meant to take that line, but it was enough to make me glad the son he wanted was never born.
