He could leave the ultra-orthodox world entirely, of course. It is possible and in Israel there are even NGOs that specialize in helping people who have left that world and are in need of support and a social circle. This could mean going completely secular, but it could also mean a different degree of observance (like being a religious orthodox Jew rather than an ultra-orthodox one. May sound similar, but those are totally different ways of life. And while the two terms are broad, there's actually a whole spectrum of religiosity in religious circles of orthodox Judaism too, which makes room for many options).
Another possibility for him would be to leave one ultra-orthodox community, the one in Jerusalem, for another. For starters, even in Jerusalem there is more than one neighbourhood for ultra-orthodox Jews. And if he were to move to a different city, there are even more options. That, however, would mean still being closeted to one degree or another, so I'd like to think this is not the option he opted for.
