I think impossible love is a constant theme in the movie... a situation that repeats itself with Tomasso and Alba.
It's never confirmed, but I think young grandma was engaged with a man but eventually fell deeply in love with the man's brother, Nicola, which I venture was gay, just like Tomasso was.
I think they even mention Nicola never married.
As it was an impossible love, instead of ending her life, she decided to move forward with it by having Nicola as a brother in law - the closest she could ever be to him - if not a husband. They become life companions and soul twins beyond sexuality or marriage. A loose cannon I suppose.
