So the crash was 100% his own fault, right?

There was no mechanical failure, no sabotage, no-one running him off the road?

He just lost control?

But why did the car seem to accelerate, even in the version of the scene near the end of the movie? (Not the one where he recovered control; the one just before that).

Did he panic and hit the accelerator instead of the brake?
