SPOILERS AHEAD

Sessomatto (1973) is an italian film comprised of 9 comedic episodes. In one of them, we see a guy, having just arrived in Milan, looking for his long lost brother. In the meantime, he meets a transexual prostitute and they fall in love. Apparently, he moves in with her, but soon realizes that she is his brother. Shocked initially, they appear to agree to continue their romantic relationship in the end.
