Missed the end, please help....+, the casting

My DVR missed the end, I got to the point where Rachel is fighting with the doctor in the bathroom and he's just stabbed her with the scissors.

I'm also a bit lost about the first patient she visited, who only claimed to be the doctor. What was his motivation? Was he a diversion arranged by the real doctor?

I was going to start a thread about the odd casting, looks wise, too but I see there are at least a couple here already. I didn't find it confusing (well I did for a minute because I'm horrible with names even in movies), but the older versus younger David's huge difference in look was certainly very distracting. Older vs younger Rachel was a good match though.
