Eh. My idea:

The opening of this was very good, where Rambo feels remorse over not being to save everyone in the flood.

A powerful Mexican cartel comes to a small town across the border in Arizona. There, they are hunting down one of their top operatives who had turned on them to the Federales. The man escapes to America and holes up in that small town.

The cartel cuts off the town's communications, and Rambo is there when he doesn't hear from his niece. With no help coming, and the cartel willing to kill anyone and everyone in the town who won't give up the man they are hunting, it is up to Rambo to kill off the cartel and save the townspeople.
