shareI thInk he just got sloppy and missed something. The whole time it's hinted at that Hoffman isn't as smart/meticulous as jigsaw. The look on his face when they wheel him out still alive showed that Strahm wasn't supposed to live.
shareIt can be interpreted both ways, but 1) as the OP said, it wouldn't make much sense to miss it among everything else, and 2) the writers confirmed at a Q&A that it was left there on purpose, to give him a chance. (I don't have a link to this atm, but there is another thread about it here somewhere)
shareHoffman had already made the mistake of making a Jigsaw trap without giving the victim a way out. Heck it's what got him into this whole mess in the first place. Leaving the pen in Strahm's pocket was the "way out".
