Does Phil McGraw have a hearing impairment or was he using wireless?

Todays show featured "Dr" Phil and his new book. I noticed he had something sticking out of his ear the entire show. Does anyone know if he has trouble hearing or if he was receiving lines through a wireless ear bud?
