Going from memory here but why did her bf look extra shocked as if he was seeing something that should be impossible to see like someone getting sucked into hell?
The movie assumes that spiritual events are not necessarily "impossible to see". In other words, Clay saw a vision.
Also, why not blood on his face from her being struck by the train?
Why would there be blood on his face? The impact would carry any splatter, forward, and the body of the train would shield Clay from any splatter than somehow went backwards.

Yes. He sees the helldragging. He sees a vision. As did San Dena ("I lost a boy's SOUL to be Beast").
