In Sickness and in Health:-spoilers

So if she was the killer in the note the whole time then why did she act so afraid all the time and call her brother about being scared?

I could see her calling her brother and being scared because she figured someone knew her secret, but why act afraid of her husband?

I'm also assuming her brother wanted to give him the note because he didn't want to lose her. But wouldn't exposing her as a killer possibly get her caught and he would lose her forever and maybe get caught himself?

She was scared that the bridesmaids had read the note and told Carlos that she was a serial killer. A guilty mind can make you very afraid and paranoid that everyone is out to get you for your crimes.

The brother tried to give Carlos the note to save him from his new bride. Her brother was clearly terrified and also retarded or just driven insane.

I think she was scared because she really thought that note was for her and she did in fact marry a serial killer. She was finally at ease when she realized that Carlos' secret was that he had dinner with another woman, and not that he was a killer. Once she found out his secret she realized that note was meant for Carlos as a warning. As for her brother, I think he was secretly terrified of her and disguised himself as a woman to keep from getting caught for warning Carlos. When he heard the message he realized his attempt had failed. They probably lived together or she just stored the bodies there because she trusted him, and he was too afraid to speak out.

That isn't what i got out of it. I doubt she though they were both serial killer. She just know someone was on to her but didn't know who.

pearlofgreatvalue is correct, she is worried that her bridemaid told Carlos what was in the note. She keeps asking "What did Ruthie tell you?" She runs from Carlos because she is afraid he knows her secret.

I'm still unclear who the brother intended the know for. Did he want his sister to freak out and cancel the wedding? Or, did he really want Carlos to know? Maybe because then his sister would have to kill her husband because he knew?

The brother disguises himself and gives the note to the priest, with the intention that the note will be delivered to CARLOS. However, remember that the priest has VERY POOR ears, so he can't hear very well. Thus, he misunderstands the mysterious "woman" and gives the note to the bridesmaid, who then assumes that the note is for the bride and gives it to her.

The bride is afraid of Carlos because she thinks that the note is for her, warning her that he is a killer. Even though she is a serial killer herself, she's still afraid of him because it's not like she wants to die. This is why she is not relieved until the end, when she finds out that Carlos' secret was that he had dinner with another woman.

Her brother is against the wedding because he knows his sister's secret, and is afraid that she's going to kill her husband, partly because he wants to protect her, too, I think. She trusts him enough to hide the body at his place, and he is happy when he hears the voice mail because he thinks that she has called off the wedding.

I just rewatched the episode (hadn't watched it since it first aired) and enjoyed it although it makes me think, is the brother also a serial killer. When he arrives at home, we see bodies and heads all around. One of them appears to be that of a young Asian man (possibly Carlos' friend) and another of an older gentleman (possibly one of Carlos' uncles or the Priest). I could be wrong about those but it was just something I observed (wanted to watch the episode since I have been getting into Psych and knew James Roday was in the episode, but not Maggie).

If those were the heads of who I mentioned above, when exactly did it take place?
I am probably wrong, but why bother showing close-up shots of the heads in the jars.

logic can not save this episode...as soon as she saw the note she would of know it was for him...dumb dumb dumb

i agree, that was totally *beep*
the note said "the PERSON you are about to marry.. blah"
why wouldn't anyone write "the MAN you are about to marry"...
that episode (and fear itself in general) has got to be the worst storytelling in a long, long time.

Read a very similar short story. No wedding, but a guy was warned by his new girlfriend's brother to stay away from her, and he would hurt ex boyfriends to prove he wasn't kidding around, so naturally the exes would stay away from the girl.

Turned out the gf was a serial killer and the brother figured it out, but he still loved her. He would try the tough guy routine with new boyfriends to actually save them, and if that didn't work he'd inflict bodily harm on them so they'd get the message.

Now THAT would make a fine storyline and prepare the way better (or at all) for the same twist. As the episode went, it was just a good story with a stupid, nonsensical ending. Too bad, because after having been warned that the series wasn't as good as Masters of Horror I watched 'Spooked', 'Family Man' and this one in a row (plus 'The Sacrifice', which was a bit meh) and was pleasantly surprised, considering it better than MoH's 2nd season in fact, till this stupid "twist" came. Now I fear the following eps might follow the path of shoehorning a downer twist ending even if it takes all sense away from the ep with it...

I know this is years late, but some of the comments are way too far off.

The bride (Sam) mentioned at the beginning that her brother wasn't there because he was upset about the wedding, specifically because he didn't want to lose her. He knew that her getting married would mean that she'd move out. So clearly they live together, and it sounds like they've basically been inseparable since their parents' death years ago.

So her brother attempted to give that note to the groom (Carlos) to scare him off so that the wedding wouldn't happen and Sam wouldn't move out.
Keep in mind, Sam and Carlos barely know each other anyway, so they likely wouldn't know if the other had a dangerous past (or present).

And with an anonymous note that simply says, "The person you're marrying is a serial killer," it's not like Carlos could even do anything about it legally, so turning her in isn't an option.
It's just enough to scare someone off but not enough to actually get the person into legal trouble, ya know?

Anyway, here's where I'm confused about it.
So yes, the "person" part definitely gave it away, so I knew the whole time that the note was meant for Carlos. So it made sense that she would be freaked out, knowing that someone out there knew her secret and was trying to tell Carlos, etc. And that explains her call to her brother, saying that something had happened and she was scared.

So I was confused at the very end when Carlos tells her about the dinner with the other lady, and Sam looks down at the note again and seems to only then realize that the note wasn't meant for her. Like it seemed clear that she knew that from the very beginning, why else would she have been so weird?

I mean, I feel like if they were both serial killers, they'd probably be more likely to team up together as opposed to killing each other (or they'd at least be more understanding and bonded through that shared interest).

So the idea of her being scared that he might also be a serial killer is just kind of stupid.

I definitely saw it as her being scared about him finding out. Even if he couldn't legally turn her in or anything, he could easily run away and leave her heartbroken.

The stuff with the uncles seemed a bit unnecessary. But while one was telling Sam about Carlos' parents mysteriously disappearing decades ago, and how that could've really ruined him (setting up the serial killer foundation), the audience also has to remember that Sam's parents are also dead (though I don't remember if it said what happened to them, but I assumed that Sam and her brother killed them).
I don't get why the uncles had the weird nods and smiles, but like I said, there seemed to be a lot of unnecessary red herrings with them.

Anyway, another reason that Sam was acting scared of Carlos could've been because, quite frankly, he seemed like a raging douchebag for a bit there. Clearly they both have some emotional problems they need to work on, things that the other doesn't see on the surface, because they've only known each other such a short time.
So he's not a serial killer, but he seems to have a lot of red flags in general.

And to reiterate, the brother isn't scared of Sam, or worried for Carlos. He simply doesn't want to lose his sister, because they have an unhealthy murder-y relationship that he wants to maintain 'til death parts *them*.
