Ending? (spoilers)

So the old lady goes back to the swamp at the end. He touches a creatures hand and looks at it's head. I don't get it. Is it still alive? Is there another creature? What secret is she hiding.

I watched the first 20 minutes and probably the last 45. I spaced out in between because it got boring.
