or combined with paranoid schizophrenia?
cause he said he had his "thoughts" to himself, without getting any funny looks. possibly referring to the fact that he realizes that he hears voices and just refers to them as "thoughts" to just sugarcoat it.

Im going to guess that maybe it was both paranoia and schizophrenia. Thats why he kept saying things about "his thoughts". Thing of it is, there are really paranois and/or schizophrenic people out there that arent in institutions, so why is Dave?

Paranoia is not a diagnosis. Schizophrenia is, but who knows. "Paranoid Schizophrenia" is a type of schizophrenia.
And not all schizophrenics "hear voices" or hallucinate. Simply being excessively paranoid and believing things "without basis" (delusions) can be an indicator of schizophrenia.
Of course, I also believe that the field of psychiatry is, or the most part, a steaming pile of *beep*
