On the official soundtrack there are different variations of the same tune, so it goes by different names.

-The part where Mika plays one of Daigo's father's records is called "On Record".
-The part where Daigo is playing his cello at Christmas and leads into a montage is called "Ave Maria."
-The part where Daigo is confronting his father is called "Memory."
-The ending credits, simply titled "ending"

The only variation that is in the movie and not in the soundtrack is the part where Daigo plays his cello late at night after he makes love to Mika. It is distinctly different from "On Record" because it starts off with the cello as a solo and then is followed by the piano

I think the tune is officially called "Memory" outside of the soundtrack.

The main musical theme of this movie is reminiscent of the song "Don't Leave Home" from Dido's beautiful album "Life for Rent," which came out in 2003.
