I was thinking Eastern Shore area maybe....lots of pretty isolated, conservative little towns with their fair share of "rednecks" as Lindsey called them....

The movie was filmed in Perryville, best known to the rest of the state as being (a) an exit on I-95 between Baltimore and Philadelphia and (b) home to an outlet center located at that exit. It's not as affluent as the DC and Baltimore suburbs but not an isolated backwoods village, either.

Like the OP, I didn't think the story fit the state very well. This crazy town, with its one evangelical church (and not a Methodist, Episcopalian or Catholic in sight, apparently), was too white to be on the Eastern Shore, too flat to be in Western Maryland, and too rural to be in the central part of the state, which is entirely urban and suburban.
