To me it seemed more than "fooling around," Judging by the flash backs and the way they were presented, it seemed like the doctor had genuine feelings for the guy but couldn't marry him.

Like I said, seemed like he cared but couldn't as another person mentioned due to his reputation.

To continue his act of vengeance. No way was he going to go down unfinished. I noticed the scene where Tawaan, during her final moments was poking at Nook's belly, possibly marking the first act of unfortunate series to come.
