The way I see it, she died (probably due to a heavy asthma attack after seeing the bracelet on Jay's arm). Her room being empty and her mother saying she's listening to her favorite song is probably because her mother can't get over the death of her daughter.
Keep in mind that Rain, in her own time, had no/little friends (not clarified very much, but she was teased at school and no one stood up for her) and her mother thought she was going insane (hence the medication). When even Jay seemingly abandoned her, it all became too much for her. The way Jay's father speaks about her, also makes it seem as if she has died.
In the very last second, right before the piano is crushed, Jay managed to play the piece fast (indicating he traveled back in time). The first one he saw was Rain and that is where the movie ends.
So now I wonder a few things, as this ending is not a very happy one.
- If the time traveled forward/backward is relative, then if Jay plays the score slowly, he will travel towards a time where there is no piano (is this even possible?).
- If he returns at the exact moment when he disappeared from his time period, upon his return he will be crushed.
Assuming he can't return to his time, he will forever be stuck in the past where only Rain can see him. Still no one will believe Rain, because they can't see Jay. Everything that got clarified to other characters in the film was for naught, as in the past this information is obviously not available.
As with all time-travel plots, it creates a few interesting twists. For instance: What will happen when Jay lives in the past for 20 years? Will his 'real' self (since there are two Jay's then) eventually travel back in time as well for Rain?
Anyway, the movie was very nice although the ending could've gotten a liiiittle bit more clarification.
