Although it is clear that sleeping with Bebe was the only way to ensure the abortion went ahead, is it not slightly ironic that presumably no contaception was used and so Otilia risked going through the whole process again but this time for her own abortion.

And for the record I think it was presented as both of them had to sleep with him. Hence the fact that Bebe told them to decide which one of them is to go first.

It's also hinted that Otilia might already be pregnant before that. You know, the puking, the nosebleeds... But that's been discussed enough.

I believe that Otilia used having her period as an excuse. She used it as an excuse to get out of whatever 'test' (which has been debated as to what kind of test it is) earlier in the movie, and she doesn't mention being on it until it becomes clear that he wants sex and that it might be a way out of having it. I don't believe that she was actually on her period at any point in that day.

I never thought of it this way, perhaps you are correct but I think differently. I think Gabita said Otilia was on her period so that Bebe would know she wouldn't get pregnant from the sexual acts. I don't think it would have mattered to Bebe, period or not, because in my eyes I think Bebe pressured that situation to happen from the get-go.

If Otilia had her period, there would have been no need to wash out her vagina after sex with Bebe. And I don't know if tampons were available in Romania back then, but she wasn't using a sanitary napkin.
