I have read some great theories up here about this flick. Bottom line, George was at this asylum from the beginning of the movie. He was a patient. "First time on this side?" they asked him on occasion throughout the movie. He was a less crazy, lol, lunatic from the "a" wing of the facility. Remember the 2 doors at the end of the corridor? He got a job in the kitchen as a reward for good behavior. All of the rest is just for Hollywood. Any questions? Loved it btw, first post up here, so hate if you need to!

Aha so you mean that all that stuff about a girlfriend and being in a rock band was just hallucinations from his crazy mind?

The fact that he's in the kitchen and shown outside the Asylum in the same portion of the movie, when everything is still normal, leads me to believe it's more straightforward than that. At the end he's just suffering from severe PTSD and reliving the nightmare in his mind over and over. His girlfriend wouldn't even be a character if he was locked up the entire movie.

i agree with the above;
the traumatic events, possibly also in combination with all the gas he was breathing, could have affected him and his brain so yes, he went insane/ got a severe case of PTSD.
