One decapitation (off screen).
One in-cell torture with screaming (off screen)
One spear impaling
Seven head bashings.
One eye gouging
One nose bitten off
One stove-top incineration
One skin-peeling with potato peeler

Did I miss any?

This was one serious Cuckoo's Nest.
