So let me get this right, he did a threesome with his niece?

That was his niece he did in the pool right?

I'll just answer your question as serious, and not a joke...

Josh, Rusty, Amelia, and Josh's mother are all ghosts. Josh is the only one who does not realize this, in part because his brain-dead body is still alive.

Every scene in which Josh or Rusty are with other characters, the other characters ignore them because they cannot see them. Lucy's mother seems to sense when her brothers spirit is around, but still no direct interaction.

So only the two girls were actually at the bar and the pool, Josh and Rusty are watching as spirits, and interpret the girls ignoring them as indifference as part of Josh's self delusion about his status.

Josh's mother wasn't a ghost. She's shown at the end of the film in the hospital just as he flatlines.

I believe she is a ghost. She kisses him goodbye and is nowhere to be seen right afterwards by the nurses and Lucy. Also I believe Marcy mentions to Lucy that her grandmother was a medium too and talks of her in past tense.

Just because you don't see her doesn't immediately mean she vanished into thin air (or in this case confirm she was a ghost). How often have you seen any drama with a hospital scene where the doctors and other attending medical practitioners actively move non-personnel out of the room so they can attempt to administer life saving procedures?
