Was Marni dead when Nathan cut baby Shiloh out of her body?
Or was she slowly dying when he did that C-section on her?

I believe she was dead, you can see her heart monitor flat lining before the c section.

from the dialogue it seems she wasn't dead or past saving but only one of them would survive so he chose the baby.

I think she was still alive. In the song Night Surgeon when the genterns say "Remember what you did to Marni", Nathan responds with "I remember ever dying whisper, every desperate murmur".

Which could also mean that she told him to do it.
That combined with the line, she was past saving, makes me think she wasnt dead yet.
