Sorta. The Sam character is based on Bo Barrett's real-life wife, Heidi Barrett. Heidi is a renowned and sought-after winemaker in Napa Valley. She comes from a winemaking family, and several of her wines have received 100 points from wine Spectator.
Did she actually have open heart surgery or was that a completely pointless add-on for dramatic effect?
